what you need

1/3 cup light (10%) cream
2 Tbsp. light corn syrup or honey
200 g (1/2 of 400-g bar) Toblerone Swiss Dark Chocolate, finely chopped

make it

MIX cream and corn syrup in medium saucepan. Bring to boil on medium heat, stirring occasionally. Remove from heat.

ADD chocolate; stir until completely melted.

SERVE over scoops of ice cream.

Substitute canned evaporated milk for the light cream.
